Payment
Please discuss any questions about financial matters before the session
to avoid any miscommunication. Payment is due upon services being rendered.
The Raffa Neuromuscular Treatment Center accepts cash or personal
check only (no credit cards are accepted).
Missed Appointment / Cancellation Policy
For scheduled appointments we set aside this time specifically for you,
therefore 24 hours notice is required if you must cancel or
reschedule your appointment. You will be asked to pay for appointments missed or cancelled without the required 24 hour notice.
Motor Vehicle Accidents / Workers Compensation Cases
Physician referral is required (but does not guarantee authorization
for treatment). Authorization through the proper channels is required before scheduling and appointment. A lien of financial liability is required upon the acceptance of an motor vehicle or workers compensation case. All attorney information and/or case management information is required prior to scheduling an appointment.
Insurance Coverage
The Raffa Treatment Center does not participate in any third-party billing, Please check with your insurance carrier before scheduling an appointment. If your individual insurance carrier covers the cost of your therapeutic treatment, the transaction of reimbursement will be solely between you and your insurance company. The Raffa Neuromuscular Treatment Center will provide you with a receipt of payment, but submission is your responsibility. Full and/or partial payment is between you and your insurance company.
